We have a career-enhancing opportunity for you to work as an Independence Co-ordinator. We offer the foundations of a career where you can make a real difference to adults with care and support needs in the local area. You will be working within a multi-disciplinary independence team which includes Social Workers/Social Care Assessors and Occupational Therapists with a clear focus on reablement.

The position will be based from Crayke House, Easingwold and will cover Easingwold and Thirsk areas. The area is supported by excellent transport links by road including the A1(m) and A19. There will be a requirement for some weekend and bank holiday working.

You would be joining us at a pivotal time for both North Yorkshire and the County Council's development as we take forward North Yorkshire’s 2020 plans and work with partners to develop a distinctive public health and adult social care agenda. This aims to support more people to live longer, healthier and independent lives.

As part of the Independence, Reablement and Recovery team, you will be responsible for preventing, reducing and delaying the needs of adults with care and support needs and their carers. Working with people in their own homes and communities, you will provide simple assessment advice, information and guidance, non-complex equipment, adaptations and telecare solutions. You will promote individual independence and recovery, meeting agreed personal outcomes within their choice of lifestyle.

You will have a minimum of Level 2 in Health and Social Care or QCF Level 2 diploma in Health and Social Care, with the service committed to give you the ongoing support and training to achieve Level to aid your professional development.
